Request for Reimbursement - Did you purchase something for a PTA event? Request reimbursement with this form! Please tape receipts onto the back of the request or onto a separate sheet of paper stapled to the back of the form. To make our treasurer's life easier, please don't staple the loose receipts to the front of the reimbursement request. Once you've completed the form, please return it to our Treasurer.

Don't forget -- PTA can't reimburse sales tax! 

Request for PTA Check  - Need a check written payable to a vendor? Send this completed form, along with any documentation/invoices, to the PTA Treasurer.
